Bu geli\u015fme, modern yo\u011fun bak\u0131m\u0131n temelini olu\u015fturmu\u015ftur.<\/p>\n\n\n\n<p>1960\u2019lardan itibaren hacim ve <strong>bas\u0131n\u00e7 kontroll\u00fc mekanik ventilat\u00f6r<\/strong>ler geli\u015ftirildi. 1980\u2019lerde mikroi\u015flemcili cihazlarla ventilasyon daha kontroll\u00fc ve g\u00fcvenli hale geldi. 2000\u2019li y\u0131llarda ise d\u00fc\u015f\u00fck <strong>tidal vol\u00fcm<\/strong>, uygun PEEP gibi akci\u011fer koruyucu stratejiler benimsendi.<\/p>\n\n\n\n<p>Zamanla <strong>non-invaziv ventilasyon<\/strong> (<strong>BiPAP<\/strong>, <strong>CPAP<\/strong>) ve y\u00fcksek ak\u0131ml\u0131 nazal oksijen sistemleri de yayg\u0131nla\u015ft\u0131. COVID-19 pandemisi, ventilat\u00f6rlerin ne kadar hayati oldu\u011funu bir kez daha ortaya koydu. G\u00fcn\u00fcm\u00fczde ise solunum deste\u011fi, ki\u015fiye \u00f6zel ayarlanabilen, yapay zek\u00e2 destekli ve koruyucu stratejilerle zenginle\u015fmi\u015f ileri teknoloji cihazlarla sa\u011flanmaktad\u0131r.<\/p>\n\n\n\n<p>Yo\u011fun bak\u0131mda solunum deste\u011fi, solunum yetmezli\u011fi geli\u015fen hastalarda oksijenlenme ve karbondioksit at\u0131l\u0131m\u0131n\u0131 sa\u011flamak amac\u0131yla uygulanan giri\u015fimsel ya da giri\u015fimsel olmayan y\u00f6ntemler b\u00fct\u00fcn\u00fcd\u00fcr. Bu destek, hastan\u0131n kendi ba\u015f\u0131na yeterli solunum yapamad\u0131\u011f\u0131 veya solunum i\u015f y\u00fck\u00fcn\u00fcn azalt\u0131lmas\u0131 gerekti\u011fi durumlarda kullan\u0131l\u0131r. Ama\u00e7, gaz de\u011fi\u015fimini optimize etmek, akci\u011feri korumak ve altta yatan hastal\u0131k d\u00fczelene kadar hastay\u0131 hayatta tutmakt\u0131r.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Solunum Deste\u011fi T\u00fcrleri<\/h2>\n\n\n\n<p>Solunum deste\u011fi temel olarak iki ana grupta incelenir:<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Mekanik Ventilasyon Tipleri<\/h3>\n\n\n\n<p><strong>1. Non-invaziv solunum deste\u011fi:<\/strong><\/p>\n\n\n\n<p>Hastaya bir maske ya da nazal kan\u00fcl arac\u0131l\u0131\u011f\u0131yla uygulan\u0131r. Ent\u00fcbasyon gerekmeden yap\u0131l\u0131r. Y\u00fcksek ak\u0131m nazal kan\u00fcl ya da non-invaziv <strong><a href=\"https:\/\/www.biosysmed.com\/tr\/biyovent-mekanik-ventilator\/\">mekanik ventilasyon cihaz\u0131<\/a><\/strong> kullan\u0131l\u0131r.<\/p>\n\n\n\n<p><strong>2. \u0130nvaziv solunum deste\u011fi:<\/strong><\/p>\n\n\n\n<p>Trakeaya yerle\u015ftirilen bir ent\u00fcbasyon t\u00fcp\u00fc ile uygulan\u0131r. Mekanik ventilasyon ya da ekstrakorporeal membran oksijenasyonu kullan\u0131l\u0131r.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Mekanik Ventilasyon Deste\u011fi Ne Zaman Uygulan\u0131r?<\/h3>\n\n\n\n<p>Solunum deste\u011fi a\u015fa\u011f\u0131daki durumlarda gerekir:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hipoksemik solunum yetmezli\u011fi (PaO\u2082 &lt; 60 mmHg):\u00a0Akut solunum s\u0131k\u0131nt\u0131s\u0131 sendromu (ARDS), pn\u00f6moni, COVID-19 gibi durumlar.<\/li>\n\n\n\n<li>Hiperkapnik solunum yetmezli\u011fi (PaCO\u2082 > 45 mmHg):\u00a0KOAH alevlenmesi, n\u00f6romusk\u00fcler hastal\u0131klar.<\/li>\n\n\n\n<li>Kardiyojenik pulmoner \u00f6dem<\/li>\n\n\n\n<li>Postoperatif solunum depresyonu<\/li>\n\n\n\n<li>Travmalar, sepsis, metabolik durumlar<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Mekanik Ventilat\u00f6r Nedir?<\/h2>\n\n\n\n<p>Mekanik ventilat\u00f6r,\u00a0<strong>solunum yetmezli\u011fi<\/strong> geli\u015fen hastalar\u0131n solunumunu desteklemek veya tamamen devralmak amac\u0131yla kullan\u0131lan, pozitif bas\u0131n\u00e7la akci\u011ferlere hava g\u00f6nderen medikal bir cihazd\u0131r. <strong>Yo\u011fun bak\u0131m<\/strong> \u00fcnitelerinde, ameliyathanelerde ve acil servislerde kritik hastalar\u0131n solunum fonksiyonlar\u0131n\u0131 ge\u00e7ici olarak s\u00fcrd\u00fcrmek i\u00e7in kullan\u0131l\u0131r.<\/p>\n\n\n\n<p><strong>Mekanik ventilasyon<\/strong>, soluk almay\u0131 taklit ederek v\u00fccudun oksijen ihtiyac\u0131n\u0131 kar\u015f\u0131lar, karbondioksit at\u0131m\u0131n\u0131 d\u00fczenler ve solunum kaslar\u0131n\u0131 istirahate al\u0131r.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Mekanik Ventilasyonun Temel Ama\u00e7lar\u0131<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hipoksemiyi d\u00fczeltmek (oksijen d\u00fczeyini art\u0131rmak)<\/li>\n\n\n\n<li>Hiperkapniyi azaltmak (karbondioksiti atmak)<\/li>\n\n\n\n<li>Solunum i\u015f y\u00fck\u00fcn\u00fc azaltmak<\/li>\n\n\n\n<li>Akci\u011fer koruyucu ventilasyon stratejileriyle komplikasyonlar\u0131 \u00f6nlemek<\/li>\n\n\n\n<li>Spontan solunumu destekleyerek hastay\u0131 solunum cihaz\u0131ndan ay\u0131rmaya haz\u0131rlamak<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Mekanik Ventilat\u00f6r Nas\u0131l Kullan\u0131l\u0131r?<\/h3>\n\n\n\n<p><strong>1.\u00a0Haz\u0131rl\u0131k ve Hasta Se\u00e7imi<\/strong><\/p>\n\n\n\n<p>Mekanik ventilasyon genellikle \u015fu durumlarda ba\u015flat\u0131l\u0131r:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Akut solunum yetmezli\u011fi<\/li>\n\n\n\n<li>ARDS (Akut Solunum S\u0131k\u0131nt\u0131s\u0131 Sendromu)<\/li>\n\n\n\n<li>KOAH alevlenmesi<\/li>\n\n\n\n<li>Sepsis, travma, beyin hasar\u0131 gibi genel durum bozukluklar\u0131nda<\/li>\n\n\n\n<li>Cerrahi sonras\u0131 solunum depresyonu<\/li>\n<\/ul>\n\n\n\n<p>Hasta ent\u00fcbe edilerek trakeaya endotrakeal t\u00fcp yerle\u015ftirilir. Bu t\u00fcp, mekanik ventilat\u00f6re ba\u011flan\u0131r.<\/p>\n\n\n\n<p><strong>2. \u00a0Mod Se\u00e7imi<\/strong><\/p>\n\n\n\n<p>Mekanik ventilat\u00f6r farkl\u0131 \u00e7al\u0131\u015fma modlar\u0131nda ayarlanabilir:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Kontroll\u00fc Mod (AC \u2013 Assist\/Control):\u00a0Cihaz her solunumu ba\u015flat\u0131r ve kontrol eder. Hasta katk\u0131 sa\u011flamaz.<\/li>\n\n\n\n<li>Destekli Mod (SIMV \u2013 Synchronized Intermittent Mandatory Ventilation):\u00a0Hasta ara s\u0131ra spontan nefes alabilir; cihaz gerekirse destek sa\u011flar.<\/li>\n\n\n\n<li>Bas\u0131n\u00e7 Destekli Ventilasyon (PSV):\u00a0Hastan\u0131n her soluk giri\u015fine cihaz bas\u0131n\u00e7 deste\u011fi verir.<\/li>\n\n\n\n<li>CPAP:\u00a0Spontan solunum yapan hastaya sabit pozitif bas\u0131n\u00e7 verilir.<\/li>\n<\/ul>\n\n\n\n<p><strong>3. Monit\u00f6rizasyon<\/strong><\/p>\n\n\n\n<p>Ventilasyon s\u00fcresince \u015fu parametreler yak\u0131ndan izlenir:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>SpO\u2082 (Oksijen sat\u00fcrasyonu)<\/li>\n\n\n\n<li>End-tidal CO\u2082 (etCO\u2082)<\/li>\n\n\n\n<li>Tidal vol\u00fcm ve dakika ventilasyonu<\/li>\n\n\n\n<li>Solunum mekanikleri (kompliyans, diren\u00e7)<\/li>\n\n\n\n<li>Alarm sistemleri\u00a0(y\u00fcksek bas\u0131n\u00e7, d\u00fc\u015f\u00fck vol\u00fcm, diskonneksiyon vs.)<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Ventilasyonda \u0130zlem ve De\u011ferlendirme<\/h3>\n\n\n\n<p>Ventilasyon etkinli\u011fi ve hasta durumu \u015fu yollarla izlenir:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Arteryel kan gaz\u0131 (ABG):\u00a0PaO\u2082, PaCO\u2082, pH<\/li>\n\n\n\n<li>Pulse oksimetre (SpO\u2082)<\/li>\n\n\n\n<li>End-tidal CO\u2082 (etCO\u2082)<\/li>\n\n\n\n<li>Solunum mekani\u011fi (kompliyans, diren\u00e7, Pplat)<\/li>\n\n\n\n<li>Alarm sistemleri:\u00a0Bas\u0131n\u00e7, hacim, ba\u011flant\u0131 kopmas\u0131 gibi durumlar\u0131 bildirir<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Komplikasyonlar ve Dikkat Edilmesi Gerekenler<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Ventilat\u00f6r kaynakl\u0131 akci\u011fer hasar\u0131 (VILI):\u00a0Vol\u00fctrauma, barotrauma, atelektaziden korunmak i\u00e7in d\u00fc\u015f\u00fck tidal vol\u00fcm ve uygun PEEP kullan\u0131lmal\u0131d\u0131r.<\/li>\n\n\n\n<li>Ventilat\u00f6r ili\u015fkili pn\u00f6moni (VAP):\u00a0A\u011f\u0131z bak\u0131m\u0131, ba\u015f\u0131n 30\u201345\u00b0 yukar\u0131da tutulmas\u0131 gibi \u00f6nlemler \u00f6nemlidir.<\/li>\n\n\n\n<li>Hemodinamik etkiler:\u00a0Y\u00fcksek pozitif bas\u0131n\u00e7, ven\u00f6z d\u00f6n\u00fc\u015f\u00fc azaltabilir.<\/li>\n\n\n\n<li>Diyafragma atrofisi:\u00a0Uzun s\u00fcreli tam destek modlar\u0131, solunum kaslar\u0131nda zay\u0131flamaya yol a\u00e7abilir.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Ventilat\u00f6rden Ay\u0131rma (Weaning)<\/h3>\n\n\n\n<p>Hasta yeterli solunum g\u00fcc\u00fcne ve gaz de\u011fi\u015fimine ula\u015f\u0131nca, ventilat\u00f6r deste\u011fi kademeli olarak azalt\u0131l\u0131r. Weaning s\u00fcreci:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Spontan solunum denemeleri (SBT)<\/li>\n\n\n\n<li>Bas\u0131n\u00e7 deste\u011finin azalt\u0131lmas\u0131<\/li>\n\n\n\n<li>FiO\u2082 ve PEEP\u2019in d\u00fc\u015f\u00fcr\u00fclmesi ile ger\u00e7ekle\u015ftirilir.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">S\u0131k\u00e7a Sorulan Sorular<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Mekanik ventilasyonla oksijen tedavisi ayn\u0131 \u015fey midir?<\/li>\n<\/ul>\n\n\n\n<p>Hay\u0131r.
